New Patient Forms
Below are forms that may be required for your visit with Dr. Beth Comeau and her team. To access each form in PDF format, click the form’s title. Whenever possible, print, review, and sign the appropriate form(s) and bring them with you to our office.
Patient Medical History
– This form is required prior to Dr. Comeau seeing any patient, and is meant to inform our office of any medical history or medications that may be relevant to procedures or treatment.
Photo Consent Form
– This consent form authorizes this medical practice and individual members of their staff to use these photographs for Dr. Beth Comeau’s “Before and After” Treatment Record. Your refusal to consent to photographs will in no way influence your treatment.
HIPAA Privacy Policy
– This Notice of Privacy Practices describes how we may use and disclose your protected health information to carry out treatment, payment or health care operations and for other purposes that are permitted or required by law. Please review prior to treatment – there is no need to print.
HIPAA Consent Form
– The Health Insurance Portability and Accountability Act of 1996 (HIPAA) protects patient information. Please review and sign this form prior to treatment.
E-mail and Text Consent
– This form gives Dr. Beth Comeau’s office permission to contact you via email and text.
